I live in a tiny rural burg of less than 4,000 people. I'm on disability, which means medicare. There is exactly one place in the area to go for therapy, and they have exactly one therapist who accepts medicare (and she is only there one day per week.)

Numbers? I remember Hegemonia, where I oversaw the orbital bombardment of worlds with populations in the billion, so I'd estimate the number somewhere around a half a trillion.

I've always had a special interest, something that absorbs my free time and serves as the focus for my attention. In elementary school, it was plastic models. In junior high it was fly tying. For a while it was 19th century culture and lifestyle. For a while it was fantasy, RPGs, and then Star Trek....
