The first Saw was genuinely mind-bending and disturbing. The other 279 Saw movies were nothing but stupid splatter-porn.

I did a whole unit of study on shocking cinema for college. I think the most horrific thing I had to watch for that was the rape scene in Irreversible by Gaspar Noe.

A lot of Lilya 4 Ever by Lukas Moodysson is quite hard to watch. Funny Games is pretty messed up.

Visitor Q is one big whole mess of crazy.

"Requiem for a Dream" & "Spun". "Requiem" was a little better with the art of the film... but so disturbing... The decaying state of the heroin/pill-addicted characters is quite disturbing- though in a good way, ,actually. I believe if they played this in schools, there would be less drug use in America.
As for "Spun," it's just a crude film. Very good, though =]

End of Evangelion. If you thought the Evangelion series itself was disturbing, you haven't seen anything yet. I've even heard a rumor that at one point during the film when a bunch of drawings flash on the screen, said drawings were provided by abused children.

Grimm Love. This movies was surprisingly disturbing. As in, I was in no way prepared to be disturbed by it at all and then couldn't sleep.

That did have really crazy taboos in it, That was inspired from the fascists in mussolinis reign i don't know if it's based on real life?
