TV shows are almost always made up as they go along. Writers don't create the entirety of a TV show at once.
Couple of shows I can think of, the writers claimed they'd written story arcs for at least five seasons, but both shows were canceled after several seasons: Deadwood & Carnivale. Good shows, too!
