Put whatever you feel is relevant on the forms. If you feel it doesn't fit in any of the designated sections, there is always a "more information" page at the back.

But that is only if there isn't a box that describes adequately the help you need.. right? Most of the boxes that you can tick give a good range of answers anyway.


I dont remember that bit, maybe the forms have changed? :? Ive only kept copies of the main pages that i filled in (only 6-8 pages) IIRC a lot of it is purely physical stuff that wasnt relevant to me (climbing stairs, using the toilet etc), so i wouldve ticked no problem and skipped the page.

Which form is it? I think i might have been sent 2 forms the first time i applied, then just one on the reappication.

Then theres the IB50 (not DLA related) thats a complete nightmare :( - just spend a week struggling with it.
