(UK) Reasonable adjustments to the workplace

Hi, I am currently revising my list of possible reasonable adjustments for my employer due to some misunderstandings in the past. I am finding it very difficult to write everything down in a general manner because my profile of abilities are affected by the current environment. Has anyone else had to write up their reasonable adjustments and if so how did you decide on the format
