Being on the spectrum, I'm incapable of "selling myself" which in my opinion is just a euphemism for bullshitting the interviewer, instead I'm gonna rely mainly on honestly and hope that they see it for what it is. While my honesty approach has worked in the past, theres only so much I can say in the short space of time and since they can't read my mind, theres no guarantee that I'll adequately convey my mindset to them so that they'll see that I'll be an asset to them. With that in mind, I might as well try to display the kinda behaviour that will convey an image of professionalism to the interviewer(s). First thing I'm wondering is should I be the first one to put my hand out to shake the interviewers hand or should I only shake their hand if they offer? Should I shake their hands before I leave or is shaking their hands once at the start of the interview enough?

always shake hands before and after first meeting someone. always look them straight in the eye while doing it. always let them know it is/was good meeting them, and try to have a smile on your face too.

Make a list of all the potential interview questions, some general, some relevant to your field.
Come up with answers, research where necessary.
Remember you can steer the conversation by leaving little hints that cause them to ask a follow up questions
related to your strengths.
Remember propper eye contact is 1/3 of the time. the other 2/3 are in the general vicinity or other people.

PROACTIVE PRACTICE PRACTICE, with a photo of a person/real person to practice eye contact at the same time.

You should absolutely initiate the handshake. It sends a signal that you are assertive, pro-active, and so forth. You should avoid anything that connotes passivity. As far as the parting handshake, I don't know. I think you'd have to be careful not to do it too soon or too late (IOW, you need to be able to tell if the interviewer is "done with you" and wants the interview to end, or not.)
