Should I put in for a stress leave at work?

I've been having a lot of problems with insomnia and memory loss problems lately, and it's been causing me to perform poorly at work and make mistakes, not being able to remember or keep track of things.

Today I got much more stressed about it when my boss came over and told me it's becoming a worse problem, and that I've been warned before about, and that this is my final warning. So I thinking final warning is pretty serious.

I got stressed out about it and thought maybe I should put in for a stress leave after that, based on my friend's suggestion if I can do that? Does anyone know how that works likely?

I vented to my gf about it over text messages on my break. She got so worried about me and thought I was going to snap, so while I was working later, she came down to see me and kept insisting that she take me to the hospital cause something is wrong with me. She kept insisting and co-workers over heard, but I had to talk her out of and get her to leave so I could go back to work cause I thought I was thin ice already.

So if I should put in for a stress leave, would her coming down and saying that work against that, or for that, or how will it affect it do you think?

A little forewarning.

I had a coworker go on stress leave last year. Being his union steward, I put in some grievances later on in regards to the stress leave he took. I can't go further in details due to the grievance procedure and confidentiality on the grievances part. What I do remember and can tell you was his attendance callendar read that his stress leave was unpaid leave.

Bottom line, if you want to take stress leave, it likely will end up being unpaid. You may have to find another job, or you can find a way to ask your employer for a vacation.
