Free Roaming War-torn Countries
Only the Mercenaries series and the new Farcry game has done this. Come on people, I want to explore the area not go down some set path through the river and over the woods to grandma's house. I don't MGS4, because despite the size of the map, you can't explore it that well.

Any Historical Background Other Than WWII
These games are definitely non-mainstream because now e have to actually read to find out what's going, Oh no! Holy Marketing Disaster In Action, Batman!

The Subway
We always go with the sewer and the train, but what if you mixed the two, you get a damn exciting sequence that's what, its too bad most games mess up subway scenes, to be even cooler, you could play Riding On The Metro by Berlin in the background.

Cities Outside The US
Where's Grand Theft Paris or Sicily? I'd rather be jacking cars in a foreign land than be subjected to the same crap again and again. We'd like to be killing random
Frenchmen in front of the Eiffel Tower, I'd even take Grand Theft Moscow, if it meant something new to the world of sandbox, well, hey, we could go with the middle east, see, there's a world of possibilities.

Inside People's Minds
Only Psychonauts was this bold, to totally flip the mind-reading process on its head, just play and you'll see what I'm talking about. Most of the time, we were inside the world of dreams in trippy sequences that were essential small breaks between the action, Psychonauts made that the entire game and then some.

Airport Levels
This is a direct result of 9/11 PC BS. Who doesn't want to shoot up the same perverts who think grabbing asses and patting crotches saves us from terrorism.

Any City In The Clouds
Most games with Cloud Cities give you limited space to roam, mostly restricted to evil lairs, military bases, and factories, and docking bays, but what if you go anywhere in this heavenly metropolis?

Canada
Can you name one game set in Canada, didn't think so. What's the deal, is Canada really that boring, with all the stereotypes you think any game developer would have a blast with it, but nope, no maple syrup power-ups for me. :(

You should become a game developer. I'd buy your stuff.

Games based on classic fairy tales.

Free roaming little red riding hood with a usable horse?

Tom Thumb stealth game?

They usually have to be violent or weird but then most fairy tales are.

Jak (Jack is too boring) and the Giant Beanstalk of Terror?

Didn't somebody make a game of Alice in Wonderland a while back with Alice as some sort of creepy psycho demon child thing creature? Like, i dunno, six or seven years ago? I can't even remember what platform it would be on, it just rings a bell. A faint one.

Re: Red Riding Hood - has to be the version from Roald Dahl's Revolting Rhymes, that was my absolute favourite when I was a kid -

"The small girl smiles
One eyelid flickers
She whips a pistol from her knickers
She aims it at the creatures head
and Bang Bang Bang she shoots him dead"

Best line ever. She appeared in his version of 3 Little Pigs too. I would totally play a game where she was the hero. Yeah.

Yes it was called American Mcgee's Alice. It was only on PC, I think.

Tribal village. Does anyone know of any games where you play as a tribesman?

Anywhere in Australia.

Inside somebody's mind.

A space environment, where you are actually in space, with only a tenuous hold between you, and whatever you happen to be standing on at the time (Dead Space seems set to rectify that).

Anywhere in the Whoniverse (who wouldn't want to explore the TARDIS?)

I always thought it would be "cool" if places in North America would be used, at least under their "indian" names. I would love to see a third-person "shooter" in Huronia.

In Medieval 2: Total War Kingdoms you can play as the Apache in North America. You can conquer the other tribes and the Aztecs, French, Spanish etc.

Haha I'd love that. Or anything historical which doesn't automatically end up with you suiting up in armour and visiting castles, or becoming a wizard. A cool game would be being part of some nomadic tribe, either leading them or just being a regular member, having to move to where there's food, having to avoid rivals, or becoming the leaders rival, plotting a take over then becoming exiled because you failed and thus starting up your own tribe. A game where you aren't being relied upon to save an entire country or the life of royalty :P

How about the gigantic mushroom tower? Trees are overused, but the only time I saw a really good mushroom tower was in Morrowind...

The most underused locations in video games IMO:

Africa
Most of Asia (Except Japan, China)
Middle East
South America

Most video games are either fantasy or America/Euro-centric or based in Japan.
