For me, it's a tossup between "Nothing's Wrong" and "I Got You Now".

Wait. Does this include mind games?

No?

Never mind, then.

I would say it is Amnesia: The Dark Descent, even though I got bored quickly.

Actually, scrap that thought: the scariest video game was actually a single level from Vampire the Masquerade: Bloodlines: The Oceanside Hotel. It is a homage to The Shining and there are no enemies to speak of (although there are three places where you can die), but you will be paranoid regardless.

EDIT: Should have paid attention to this:

The Gamecube Remake of Resident Evil got me a lot... even watching it and sort of expecting it... it still got me.

Sora: "My friends are my power."

Ventus: "I'm asking you as a friend. Just... put an end to me."

The first Resident Evil when I was a kid. Way too young for it.

First Resi Evil dogs made me jump like crazy... I was really scared of them.
Now though fear doesn't put me off a game as it can be like a sense of thrill out of it.
Surprised that no-one mentioned System Shock 2, never played it myself but yeah... that can be scary.

Alien Isolation in VR. Never finished it, and unfortunately it was never updated to work on current hardware.

I thought battling skelley archers in minecraft as scary, also dropping into water down low! :P

anyways for true scares it would be FEAR series...pretty badass though to fight those mechas or whatever they were.

I tried condemned to, it can be jump scary.

I tried doom 3 and was sh*****g when the first few enemies/demons, they look really well done.

Doom 64 is a good deal more atmospheric and scary than the original game, and even more so in its Brutal Doom 64 incarnation. The pink demons and the Cacodemons are considerably more fearsome, and the misting and lighting effects make a difference too, not to mention the sounds and music.

Probably Vampire: the Masquerade - Bloodlines. I can't recall finding any game ever particularly scary though.

Probably Siren on PS2. Forbidden Siren for those in Europe.
