It's not just a simple "miss", though we've become used to calling it that. You're rolling against their armor class, which is a combination of their dexterity (ability to dodge your attacks), and their physical defences (armor, shield, thick skin, bones, etc.). Also, very small classes (like halflings) are harder to hit, while very large targets are easier.
There are other variables to take into account (such as +1 for having the high ground), but that's the gist of it.

Yes, I know that... In a game like Baldur's Gate or Neverwinter Nights, dice rolling works fine, but with gameplay like Dungeon and Dragon Online's, it would be better off without dice rolling.

I have yet to play any BG game, but I'm playing Neverwinter Nights 1 right now, and it's great.

It's possible, but incredibly hard. I've seen people do it thought, at least on the Bioware forum.
Currently replaying Metro 2033. Fantastic game.
