The question is "Have you ever found an undocumented glitch in a game?" but I also mean finding design flaws or things that can be abused... and having found them on your own.

I found a really cool one in Ninja Gaiden Sigma 2. As far as I'm aware nobody has posted anything about it... and I found it on my own. There's a large skeletal bone dragon boss in the intro of chapter 7. The thing is as big as a small building with the hp to match. Anyway I found a bug in it's programming that made me laugh a bit. It's not the most insane boss in the game but it usually takes along time to kill so your endurance is where the damage is dealt with this boss. Anyway when you damage him like 5% he falls on his butt so you can hit him easier (He's very tall)... but it's a very long toppling animation. I for some reason decided to throw a art of the inferno ninpo at him during this slow animation and it took like 90% of his health away... it was awesome because I then hit him a few times before he got back up and he died :)... Anyway that was about a week ago when I was playing on warrior difficulty. I now just beat him the very same way on Mentor difficulty. At first I was like what the heck?... it didn't work cuz I did it to him while he had already fallen (Did maybe 5% damage), but then I tried again the next time he was falling and sure thing the bug is to hit him during his falling animation. I think it's a funny glitch because It's kinda silly to see something so big with so much hp to just drop dead from a single strike :)

I found a lot of glitches in Tony Hawk's American Wasteland. I'm pretty sure that game was made for finding glitches! I'm not sure if I was the only one to find them, but I found a few on my own that I never heard anyone mention.

That might have been found but the developers being the time-crunched dismissive jerks they are probably just marked it "As Designed" and just let it slide.

/used to be a tester
//there's PLENTY bugs that are documented that still get through because they aren't critical issues that completely take you out of the game (ie, freeze, crash, etc)

edit: actually there are a couple that are really bad as well but they're so improbable to come across and aren't in a normal user-path that they'll just pass on it since it won't be found by +90% of the people.

Now that I think about it it seems like alot of bugs I've seen in games are based on falling animations. In Mass Effect 1... there is a falling animation that if you get knocked to the floor there's a high chance your game will freeze most of the time. I only experienced this in the arena mission dlc... it almost made the dlc unplayable >_<

And on a less bad note there's a funny affect my friend showed me in Demon's Souls to kill a certain high leveled enemy to level up fairly fast at mid levels. He showed me an enemy that normally couldn't be "Soul sucked" (An attack that's an insta kill), but he showed me to backstab it first and that would knock it to the floor. Just after it falls you cast the spell and then it'll get back up, spin around 720 degrees and drop dead... weird but cool :) Usually it would just turn around and immediately kill you if you try to just soul suck it... It's a black phantom skeleton that only occurs during pure black world tendency so it's worth a ton of experience... although I'm not sure if my friend found that or found it on youtube or something.

Just thinking of those past glitch like things and the one I found made me think maybe there's something key about falling animations... I dunno :?

I found a Dr. Breen zombie in half life 2(Zombie AI, Breen Model). It was in Ravenholm. It was in the Smod tactical modded version, so I'm not sure if it counts.

Also, the term used in industry is "bug". Calling a bug a glitch was very much frowned upon.
