THERE ARE SPOILERS INVOLVED SO READ AT YOUR OWN RISK!! !
There are many interpretations of the game.
First, is the character select. Isaac seems to have multiple personalty disorder because you have so many characters to select and the words "Who Am I?" at the bottom. Ending 11 also proves this theory.
Second, The intro and epilogue. Isaac seems to have read the bible story of Abraham and Isaac and drew a what if featuring him and his mother. and thought "this would never happen to me" but it did anyway.
Third, is the story behind the photos in ending 13 and more.
First picture: Isaac's family together.
Second picture: Maggy is also part of the family.
Third picture: Isaac killed Maggy for unknown reasons. (Maybe he was jealous his parents loved her more than him or maybe she comitted suicide).
Fourth and five picture: Parents happily took a picture, and Isaac went to tell them whatever happened (Either he killed Maggy, or she comitted suicide and the grief later made mom nuts with dispair dresses Isaac in a wig as seen in a flashback).
Sixth picture: Parents lock Isaac in his room because of what happened, blaming him.
Seventh picture: Mother rages and wants to attack Isaac.
Last picture: Isaac's dad couldn't stand what happened so he decided to leave the family. Mom's attempt to attack Isaac wasn't real, in-game fight is a representation of how Isaac battles his fear of his mom fighting him. Isaac then reads the bible (Ending 12), realizes what he did the entire game is his imagination (Ending 11) and then locks himself in the chest to die.
Fourth, the Alternate Maggie theory.
Second picture: Isaac's mother wanted a daughter so she gave him a girl's wig for his birthday.
Third picture: Isaac, feeling so dirty, hated himself and his mother for this.
Fifth picture: Isaac doesn't want to wear the wig anymore (since it attracted bullies as seen in a flashback).
Sixth picture: Isaac's locked in his room while his parents are probably fighting.
Seventh picture: Mom goes insane.
Last picture: Isaac's dad couldn't stand the madness and leaves.
