I still find the Dead Hand from Ocarina of Time really unsettling. Worse than ReDeads, in my opinion.

the first time i played that game, i was 13 so i didn't really feel "scared".
dead hand is rather creepy though. also certain rooms in the Great Bay Temple in MM feel...off for me, in a way that's hard to describe.
the Spirit Temple as well.

I put in a cheat code in Crash Tag Team Racing to turn all the park civilians' heads into chickens. It was funny at first until I attacked them and then their chicken heads exploded in a puff of feathers. All that remained were their headless bodies walking around. It was just creepy watching them following me around like a bunch of zombies WITHOUT ANY HEADS. I did a spin attack on a child and tears just flowed from its neckhole. I did the same thing to the fat guy with the ice cream cone and his ice cream just landed floating in the empty space where his head should've been. What have I done? 8O

The possessed music in the River Twygz Bed in Super Paper Mario with floating invincible hands that want to get you. Nintendo has a silly habit of sometimes inserting something scary somewhere in their rated E games from Lavender Town to Giygas to that piano in Super Mario 64.

The redeads on Ocarina of Time scared the crap out of me when I was a kid. Also, that one part in front of the Forest Temple where a gigantic moblin runs out of nowhere and charges at you with his spear. That game had a surprising amount of nightmare fuel for an E-rated title, probably explains why they upped it to an E10 for the 3DS remake.

Doom also has some pretty good jumpscares, especially this one room in one of the early levels where as soon as you pick up this key, the lights go out and a bunch of enemies come out of this closet and try to kill you. It's frightening if you don't expect it. :P

Also, any multiplayer FPS mode where you have to stand on a point to capture it without being killed, particularly the Hacker Central mode on Perfect Dark. In most games you just have to be standing within a certain proximity of the point, but on Hacker Central, not only do you have to be right up close to the computer terminal you're trying to hack, but you also have to keep the Data Uplink aimed right on it, otherwise the connection gets cut and you have to redo it. It gets really really really tense when you're waiting for the connection to finish, and there's somebody right around the corner ready to blow your head off. Of course, on other FPSs it can be pretty tense as well, but not as tense as PD.

I missed those Thief games, but when I saw this thread title, I knew I was going to mention The Forsaken level in the new Thief. That's the scariest level I've ever played. I didn't even bother making sure I found everything, I just wanted to finish it as soon as possible :)
