It sounds like the little boy died because of a lot of assumptions being made. The sad thing, is that if the pharmacist who filled the prescription, and the nurse administering the medicine had simply double checked with someone else, and asked questions before proceeding, it might have been avoided.

The medical profession kills more people than automobiles and murder, then investigate themselves.

That's pretty appalling that they managed to kill a toddler with a massive dose of an ordinary amino acid. The toxicity of it is very, very low, but they injected him with 1/8 of a pound of it! Holy cow!! !

I worked in a medical capacity at one time, and it was practically drilled into us a students, when in doubt, ASK! It's never safe to make assumptions in medical treatment. Giving a patient a sip of water can kill them under the right circumstances. It sounds to me like both the pharmacist and the nurse had doubts, but then acted on their own judgment without double checking with someone else. Either in a rush, or allowing pride or ego to take over.
