Make the appointment by email.

Type out your list of concerns prior to the appointment and hand list to doctor when you meet them.

Take laptop or phone to appointment and answer any questions by typing.

I am now more comfortable with the medical professionals in my life and can usually speak but sometimes I still have to resort to typing on my phone.

I can't go there, though.. It takes a 15 minute ritual just to take out the trash, interrupted by any signs of life.
Getting myself to another building over a mile away isn't possible. Not to mention the breakdown I'd face when confronted by clinic staff. When I said crippling anxiety I was being quite literal.

That's another day gone with no word from the doctor. I wonder how many emails she has to go through.. I wouldn't think many, doctor's can't really afford to get behind on messages, right? If I don't hear from her tomorrow I think she probably found the reception's response satisfactory.

The clinic staff are easy. All you need to "say" is "I have an appointment with Dr (whoever) at (such and such time)". Then they'll tell you to take a seat. I have done this when my mutism is bad by passing a note to staff.

Kraftiekortie makes a good point about can't you go with someone else? If not you will have to explain in your email to the clinic that you can't attend and need a home visit.
