Still state by state in the US. As it should be. There are some bastions of freedom left.

Yes, state by state. My state (Maine) has many non-smoking laws in place, however if I go over the New Hampshire border (Or, last time I did, may have changed) there is still smoking in restaurants, and you can still book a "smoking" hotel room.

I smoke rather a lot now because i am stressed a lot. I think genrally the ban is a good idea. I personally do not have a problem about not smoking in cafes and resturants- I chose not to anyway.
Not smoking in pubs will be something difficult to get used to. I suppose i will have to just go to pubs with gardens and wrap up warm.
If i was with someone who was bothered by smoking, i will not smoke whilst i am with them, thats just basic manners.
