Social Anxiety and Situational Depression. I'd say the depression is probably linked to having a combo of Social Anxiety and Aspergers. Being that I have two disorders hampering my communication things can be rather depressing.

Mental conditions: Depression, Autism, generalized anxiety disorder, schizoaffective disorder (don't agree with that one), conversion disorder, used to have panic attacks, social anxiety disorder, Dissociative Identity Disorder, dependent personality disorder and finally Internet Addiction Disorder.
Real stuff: High blood pressure, mitral valve prolapse, extreme fatness due to the psych pills, sciatic nerve problems, back problems, old age (I'm almost 25- that's a quarter of a century old!) generalized confusion disorder ( I made that one up but I do have it anyways), bad memory, and I forgot what else I got.

GID/Gender Dysphoria
Some random allergies to dust, trees, grass, etc.
Bronchitis (frequent/reacurrent but not chronic...>.>)/possibly asthma
Probably something OCD related

Phobias:
Arachnophobia- spiders
Claustrophobia- small or enclosed spaces
Cleisiophobia- trapped in enclosed spaces (different from above lol)
Merinthophobia- bound/tied up/wrapped up
Melissophobia- bees
Ligyrophobia- loud noises/sounds (though this may be more due to my sensory issues)
Not sure what to call it off the top of my head but, a fear of phones/talking on the phone/answering the phone (EDIT: I found the title! Telephonophobia  Seemed too easy...)

I also have fears that can't be classified as phobias (they aren't as strong as the ones above) like, fear of public speaking, dirt/germs, being burned, and people watching me eat.
