There are cases where people
do have to cut off shoes, yes. Having shoelaces can make this a less likely scenario though. Temperature and blood flow/circulation changes (not to mention injuries) can cause swelling in any extremity. This in turn could lead to a situation where one has to cut off a shoe or piece of jewelry.
In terms of getting feet stuck in something like a toilet: The foot is asymmetrical (and capable of bending), so it may be much easier to fit it in something from the direction of entry than the direction of exit. Friction forces will not be the same in the direction of exit. I'm not a physicist, but many a rock climber/spelunker could tell you that it may be easy to enter a cave/enclosed space with the body in one formation but impossible to exit out the same way!
