A cow worker told a story about in the south of USA, a bunch of kids in a shelter for abused kids had food poisoning, and the nearest hospital was four hours drive away, and they had fifty kids in there and all were sick, and they couldnt find any buses to take them to the nearest hospital. so they just waited it out, and the kids got better.

What I want to know is, this ridiculous story the teller claimed happened many years ago. Can a town be so small that it doesn't have a hospital, even though nearest hospital is hours drive away, and they cant find buses, but IT'S LARGE ENOUGH TO HAVE A CHILDREN SHELTER WITH FIFTY KIDS IN IT! Is it my imagination, or is this person playing a game? Do such tiny places even have kids' shelters?

Yes, it's quite possible for a town to be so small that it doesn't have a hospital----and, maybe, the closest hospital is in a major metropolitan area, that is hours, away.

It's also possible that there was a children's shelter in a small town----it could have been "covering" for several small towns, in the area, and that's why it could have had 50 kids in it.

Though the part of the United State I live in is far from tiny to the point where there are five banks walking distance from me, there are towns small enough and isolated enough that that story is definitely plausible. The cost to operate something like a hospital in a place with such a low population would not be cost effective.
