What I want to know is, this ridiculous story the teller claimed happened many years ago. Can a town be so small that it doesn't have a hospital, even though nearest hospital is hours drive away, and they cant find buses, but IT'S LARGE ENOUGH TO HAVE A CHILDREN SHELTER WITH FIFTY KIDS IN IT! Is it my imagination, or is this person playing a game? Do such tiny places even have kids' shelters?

Yes, it's quite possible for a town to be so small that it doesn't have a hospital----and, maybe, the closest hospital is in a major metropolitan area, that is hours, away.
It's also possible that there was a children's shelter in a small town----it could have been "covering" for several small towns, in the area, and that's why it could have had 50 kids in it.
