It is my understanding that there is always some discomfort and pain involved in the exam, as the speculum is used to get a sample for testing. There is no way to do it without pain on a conscious person, and they don't put you under anesthesia for the exam. Of course some docs are better than others at it, and the size of the speculum matters, too, but there will always be some pain and discomfort. The tissue inside your cervix is not hardened to the elements to any extent, as your outside skin is. Rather it is tender like the skin inside your cheeks, and perhaps even more so.
Unfortunately, this is the burden we must bear to be checked for STDs, and cancer.
