Exactly the same thing here. It is extremely difficult for me to address an unfamiliar person by name. I always phrase sentences in such a way that saying names becomes unnecessary.
Surprisingly, I have another oddity as far as names are concerned. I tend to call my family members by first names, and I call my grandfather by middle(!) name. I never called my parents "dad" or "mom", always by name. And within our family it sounds informal. Moreover, very soon all the other family members also cought this habit from me; and now everyone calls my grandfather by his middle name etc. In the last two years, however, I learned to call parents "father" and "mother" (in Russian, of course).
Also, sometimes (but not always) I tend to address my very close friends by first and middle name in plural form (very polite form in Russian language), and again, they also learned this habit from me with time and addressing each other in such a formal way became a sign of an informal conversation in our circles.
