I thought he meant it literally, but it turned out that in that situation his word "now" meant "lately." My mom was with me at the time though so she was able to translate; right at that very moment I wasn't in pain, but I'd been in lot of pain the previous days. It would've caused a lot of trouble if the misunderstanding hadn't been solved.
It's just that I learned that, to majority, the meaning of the word "now" depends on the context as an adult, so it's easy to forget it and remember what I learned as a child, which is that "now" means this very moment.
That would have confused me too actually.
