You could not have expressed your dilemma more succinctly ........ shame the lady was not more astute !
( had the brains the good lord might have endowed her with) ........ {sorry for being outspoken}.

One would think a therapist of all people would understand her patient population.
I have experienced this same situation.
You are not acting out of passive aggression but a need to understand this crazy world.
The therapist should have responded, "Yes, of course, I understand how that would be confusing. I definitely want you to keep a diary and not a planner."
Simple. Professional. Most importantly, let's the patient know she cares.
