It sounds like an exciting idea. But my oh my I just can't see it being used in nursing homes and with elderly people with Alzheimers. I can hear it now:

"why did that man turn blue? Wasn't he red before? Why is there a minus sign over his head? Wait! Why is there suddenly a plus sign over his head? He had a minus sign over his head a minute ago didn't he George? George? George- wake up!"

I can think of something more useful, a subtitles board attached to NTs which gives me an idea of their emotional state and the reason why they feel they way that they do.

It could be a little troublesome, in human traffic the girl who was raving and doing drugs all weekend goes for sunday lunch with her mother and the local priest. She is telling lies but there are truthful subtitles, very funny.

Back in the 70's there were mood rings. I had one. You wore the ring on your finger and the gem turned color according to your mood (actually, according to body heat which the manufacturers correlated with mood). They came with a little cue card to tell you what color indicated what mood. Mine was alsways wrong because I have poor circulation and therefore cold hands. But it was a fun technology. So in a sense, the technology is already there it just needs refinement.
