Okay, so I am going in for my re-evaluation at a different clinic on Wednesday. They are giving the ADIR to my father to ask him about my childhood and such. I understand there are 96 (or so) questions involved, and I am wondering what types/categories of questions he should expect. I am not asking for me, as I probably won't even be in the room. I am asking to see if we have to get some information from my mother (who was around more) as she is unable to take the time off of work to go with us. Any help would be greatly appreciated!
