What I know for sure is that Germany uses the ICD-10 in all matters official concerning the government, insurances companies and referrals which require or for another reason include the code of a diagnosis.
Occasionally, the DSM-IV-TR is apparently used for support during a diagnostic process. Whether it is used or not and how frequently seems related to the disorder a person gets tested for.
My official diagnosis of ADHD/Hyperkinetic disorder is of ICD-10 but I've been inofficially "diagnosed" with normal ADHD (ADHD-combined type it is called?) of the DSM, meaning the professional went ahead and tested me with the DSM-IV-TR criteria first before applying the ICD-10 criteria also.
