AS Assessment Questionnaire for Adult's Parents (ADI-R?)

Hi All,

does anyone know if it's possible to obtain one of these questionnaires, that they sometimes send to your parents (or conduct over the phone with them) when you go as an adult for assessment for Asperger's and your parents can't be present? I think it might be the ADI-R one but not 100% sure.

This might actually be it, but it's a sample one and I can't understand the scoring whatsoever!
