Got a form from health insurance to confirm I'm still a student.
I hate forms. They have lots of small text that I tend to ADD skip, so I accidentally filled out "only fill out if you address has changed"...
Overall, forms don't specify wanted information enough for my taste, like there is a group of 3 boxes and "fill out only if anything has changed" is only printed ONCE under one box and it is assumed that you just know the same applies to the rest. I find that rather annoying.
Also sometimes I'm really not sure what I'm supposed to fill out.
Has happened before that I've skipped entire sections, filled out stuff incorrectly, checked the wrong boxes by accident and the thought of a huge form just makes me want to procrastinate...
edit: elaborate forms are also very intimidating (information overload)

I don't like filling out forms.

It's the questions that get in the way for me.

Filling out forms is my "special interest" (if NTs have any).

Call your health insurance company and have Member Services walk you through the form. If they are trying to verify that you are still a student, their form may actually be superfluous and all you have to do is to send documentation to confirm current full-time enrollment at a recognized college or University by the registrar. But check with Member Services first and / or ask for their help.

I frequently experience panic attacks when I have to fill out forms - like full-blown limbs shaking, heart racing, can't breathe, feel like I'm getting stabbed in the chest, and dissociating from reality panic attacks. Or else I'll pop a (prescribed) pill to curb the anxiety, but then I have to have someone proofread the form afterwards to make sure I didn't fudge it up due to being medicated, which has happened before.

So yeah I really hate filling out forms.

Whenever I fill forms, I have these questions in my head:
"Do I have to be more specific?"
"Is this answer too long/too short?"
"Is this enough/too much info?"
"[Insert jargon questions here]"
"Do I have to capitalize all the letters?"
"Should I write this in a [Insert 'appropriate handwritten font']?"
"Is my handwriting readable?"

And many more.

I'm sure I drive my office's administrative assistant mad when I'm trying to fill out forms for a purchase request or some other thing. I usually have fifty questions for them on how to fill out certain parts of it because I am never entirely sure what the form is asking for. I get extremely frustrated with how ambiguous items can be on them.

I don't like forms, I often make mistakes regardless of how much prep or planning I do for each section, especially if I have to complete it quickly.
I must be organised and have all the necessary information to hand, because if I stop to look for a missing piece of information I'm likely to not return to the task for another week/etc, especially if I have difficulty finding a required detail.
