Need Help in Filling Up Insurance Claim Form

Hi,
I am father of RN - a 3 year old, who has been recently diagnosed with Autism. We live in New York, USA. I have hired a professional, who is providing ABA service for RN at home. The professional is 'out of network' in my insurance. So, I am paying for the service in full and I am supposed to submit the claim to my insurance for the reimbursement.

I am stuck in certain sections of the medical claim form (yes - that's the form that my insurance is tell me to submit). Please see the below image for the sections (highlighted), which I am stuck in. Can anyone here tell me what should be put in those boxes for ABA Therapy?

the person providing the service should be able to give you these codes. providers generally have a book to look these up in, if they don't know them by heart.

Thanks cathylynn for your reply! However, the professional, who is providing service does not have a clue regarding medical billing. She is employed by an agency, whose billing department takes of these things. However, I have bypassed that agency as they were charging extra (to be the middleman) for the same service and have employed her directly. However, I'll double-check with her to see if she can somehow manage to get these info from her agency.

i googled the codes. the diagnosis code for childhood autistic disorder is 299.0. the procedure code for ABA is 97532, to be billed in 15 minute units. might not be a bad idea to still get your therapist to double-check these.

Wow - thank you, thank you Cathylynn for helping me out!! I'll definitely have them run against the therapist. The ICD Code 299.0 will probably be changed as per the new ICD 10 standard. I'll run that against my insurance company to make sure what they want me to use.

However, these does not diminish the tremendous help that you provided me in such a short time! I spent almost a month searching for these info in different places and different people in vein. And you gave me the answer in 30 minutes! Thanks very much once again. God bless you.
