I have my evaluation in five days and I'm trying to figure out what to bring. I'm bringing a copy of my IEP paper work from K-12 but other than that, I'm not sure what to bring. Any advice will be greatly appericated and here are gifs of a kitten and puppy as my thanks.

They need statements from family about your behavior.

They called and interviewed my parents after they talked to me, so ymmv.

Bring:
* An national ID card or any certificate that shows you have the right to healthcare if it's a gov organisation that do the testing.
* Honesty.

I have heard childhood records, like teachers notes about you can be useful. But I would say unless they are asking for particular stuff, not to worry about it.

I very much enjoyed those gifs!

I would make list of things you think might be pertinent. Maybe things you like and dislike. And you might want to say that someone recommended you make a list, because they will note whatever you do to prepare for the appointment.
