...but didn't act upon it.

Today, I think my dad's girlfriend was not feeling well. Like emotionally. I felt there was something wrong. My brain went through a few things, such as comfort things like a shoulder rub or just an "Are you okay" or even a hug. I felt like I needed to do something, but I didn't actually do it.

Anyone been through this?

Haha, yes, it happens to me. Every time, I start thinking about the best way to deal with it, agonizing internally about every possible option until it is plainly too late. for it to make sense.

I go through this frequently. I identify a problem and run through a mental list of possible responses. This is how most of my social interactions go, but sometimes finding the right responses and implementing them can be hard and exhausting.

I came up with a list I go to during times of uncertainty.

"I hope you feel better."
"Would you like a hug?" (If I feel up to giving one.)
"Do you want to talk about something?"

I absolutely understand. It's like my brain indetifies the problem, it has a clear list of what is appropriate to do in the situation, however, my body or my mouth never get the message.

Brilliantly phrased Verdandi. Describes something that happens to me all the time.
It's frustrating and tiring.

I don't like being the jerk who saw something wrong and never acted upon it.

It's not too late. You could always say something like "Were you Ok yesterday?, I thought you were upset but I didn't know what to say" Even the day (or two) after, I'm sure it would be appreciated. Not knowing what to say is a perfectly valid reason for not saying anything.
