Some ideas I've had but never used:

Evil King acquires ring of wishes but doesn't realize what it is. You and your party must get it back before he discovers what it is.

Party must find a lost artifact hidden in an abandoned temple. Artifact feeds on death around it, and if it gets too powerful will become self aware. You must find it and destroy it with as few casualties as possible.

Party is imprisoned and separated in enemy country. An escape plan forms, but requires each to learn skills in another class, possibly only by whispered instruction at night through cracks in the wall or secret messages in the day.

How big a campaign?

I've run one-off games of about four sessions involving a simple tavern -> ruins -> treasure, and I have run epic games involving the origin and nature of the world.

A heroic paladin famous for saving the world becomes an evil warlord and plunges the land into chaos and despair. Why? Because his lawful good deity was corrupted himself.

The Nether Saga

In the final clash of light and darkness, good and evil, that heralded the end of the world, a new race was born: the Nethers.
The sheer concentration of opposing elements and forces in one place created a rift between heaven and the underworld, fueled by a new form of energy called Chaos Energy, which can only be created artifically by an extremely difficult spell, which requires the souls of a hero, and a Destroyer. Because the spell also requires the magician to be perfectly neutral, and stealing the souls of the dead or living is a serious offense, it is impossible to perform unless both souls were volunteers. However, the gods wanted another chance to create another world, so they in fact performed the spell with the dead soldier's souls, creating the Netherworld.
This phase in the D&D game mainly consists of adventures in the Netherworld, except with all the races replaced with their nether lookalikes, and having been granted the ability to manipulate Chaos Energy. FYI, Chaos Energy looks like red lightning.
