Inner monologue interference for those of you who have one

Have you ever felt your inner monologue go dark for prolonged periods of time? For example, it occurred to me that I may have had a cluster or other headache that was too strong that the inner monologue either wasn't going or was not "loud" enough to get get past tinnitus and the fuzzy headache in my head. I also had tinnitus that was going on in the background (could not hear it unless lying down) that could have contributed. I may have had so much pain/pressure/etc. that I was having a cold sweat halfway through the night.

What happens when this occurs?

I assume by inner monologue, you mean thoughts?
