Concentrate on the personal hygiene part as that can directly affect most aspects of your life. Social, health, work etc etc.
I think Yigeren's hygiene list is pretty accurate. Probably a little excessive in my opinion. I mean, shaving is more a preference than a hygiene thing. Other than the occasional trim, I haven't taken a razor to my face in a very long time. At least 10 years. I'm not a dirty person. I scrub my beard and I'm good. Of course, if your SO don't like it, gotta adjust. Luckily mine hates clean shaven.
Watch cleaning out your ears as it can be medically bad if done incorrectly. I used to go crazy with it, and wound up with impacted wax and got a bad ear infection. Dr. told me a q-tip should not actually enter you ear canal, but rather go lightly around the outside. Your ears can be flushed if need me using one of those baby-bulb thingys with diluted Hydrogen Peroxide.
I bite my nails profusely, so I'd never have anything to trim. So I'm not sure if that's totally necessary too often. Though if they are long and start to look yellow or have stuff underneath, best clip them SOBs.
As far as housework, paper plates and plastic utensils are your friend. I know I personally load up on them at the convenience store in NJ, Wawa. Many fast food places keep them out too, like many Taco Bells. I just grab a bunch. Some people think that's sh***y, but whatever. I take a ton of their condiments too so I don't have to buy ketchup or mayo ever.
If you dump a shedload of bleach in your toilet and close the lid and leave it for a little while, you really don't even have to scrub it. The stuff just comes off. Do it on a semi-regular basis and at least the toilet's ok.
If you do have to do actual dishes, and are anything like me and have them sitting in the sink for an embarrassingly long time, use rubbing alcohol when cleaning them. You can assure they are sterile, and film and gunk comes right the hell off. Makes it easier when you don't have to scrub for hours.
These are hacks I can get behind. Is the bleach thing safe though? Because I know bleach + ammonia = chlorine gas, and urine has ammonia in it.

Sure it is. I mean I'm sure if you dumped in a gallon of bleach and a gallon of piss together maybe. But howw the hell much do you piss man?
You could try making a to-do list, and take breaks in between tasks. Do something relaxing, like coloring or listening to slow music. Think of it as a group of little tasks rather than one big task. The same goes if you have a task that is particularly large and cumbersome: break it into pieces.

I've tried this, I usually end up getting distracted from what I'm supposed to be doing. And if I try to get back to what I need to do, I usually don't get very far.

I like this collection of "five minute" tasks from zenhabits:
http://zenhabits.net/18-five-minute-dec ... your-mess/
I also have had pretty consistent success with picking a "sanctuary" spot to start, like a section of counter or my bedside table or really just about any roughly 2' square area of my living space and I move everything out of it that shouldn't be there, clean everything that should and put it back in an orderly fashion.
This requires a secondary rule to succeed: I only touch things once [unless they get mass cleaned like dishes in a dishwasher or laundry]. So anything I pick up has to go to its home, the garbage, the hamper, the dishwasher - or one designated "whatever" box... because nothing will sidetrack me like those little maddening bits I don't currently have a place for. It's totally permissible if I stick to one and go through it occasionally (this happens... half the time or so, and when life gets chaotic grows into a stack of boxes but still I persevere!).
What I do:
1) shower every day (or after you're dirty/sweaty) - it's okay though to postpone it if you've been busy the entire day
2) before summer - pest for insects. Call a professional. From my experience those sprays against flies and cockroaches quickly fade out.
3) Perfume/deordorant.... important. Dressing appropriately to work (If you work in a zoo, or in an office... example)
4) vaccum cleaners won't ever ever ever substitude water. Seriously, throw buckets on the floor, use the shower head or throw water on the toilet unless you got like wooden floor then use little water, rag. Dry at the end. USE chloride
5) Cooking- I personally love it and good at it. INTERNET is everything. just google watever you wanna make.
6) Tools: I've got some handy useful stuff: Oil spray (doors), philips/flat screwdriver, Hammer and nails, things should be in every house. You may also keep lightbulbs or whatever source of light you've got... for spare.
7) Shopping: Make a list, always. Check everything and mark what you need. Also care for your budget.
Once a year= dentist (+teeth cleaning), eye doctor (if you have glasses), blood test (especially if you take pills)
9) constantly replace:: tooth brush (monthly), cleaning pads, pillow buy new every year or so... you'll see its condition, care for laundry, bed sheet, clothes.
10) Make friends, good terms with neighbors, and don't be afraid to ask for help. Good relationships is good )
