My very good friend was in two bike accidents in the course of 3 weeks. In the second accident he strained what I think must be his right adductor or front-groin area. There was a big knot there that he would not let me see because it was bit after a while, the pain subsided and moved to his hip...so his hip became the main location of the pain...he still has the big knot, but he says it doesn't hurt...

I am trying to figure out what might have caused it....a damaged lymph node, perhaps?...or a torn muscle....?

he is not one to go to the doctor, due to lack of funds, insurance, and due to the sensory stress of being poked and prodded by strangers...(he's AS-ish)...I have nursed him through several injuries over the course of the last year...including one previous bike-related groin injury...and would like to know a little more about what is going on with this big knot he has.

Any advice (other than my suggesting that he go to a doctor..because it is futile)...would be helpful...

well, i mean what's a doctor going to do for him anyway?

1) take Tylenol for pain but not more than prescribe - it's bad for the liver
2) he might have torn a muscle, in which case, I mean you could help him do passive range of motion exercises
3) rest the leg? use crutches? avoid active exercise?

there are clinics for homeless people, like the free health care van, and you can help him get insurance like Medicaid just for the future.

but rest and passive motion (you moving his leg slightly while his leg is relaxed) can help preserve the flexibility after the injury heals.

is there bruising in the area?

how did he fall? it is possible he fractured his hip a little, or something else, X-rays would show that.

He needs to GO TO A DOCTOR. Now. Drag him if you must but he's going.

There are probably teaching hospitals (free) in your area. They are usually attached to universities. Take him to one of those.

He needs to take care of this now, or risk a permanent injury that could torment him for the rest of his life.

I have had good results with do-it-yourself myofascial therapy for various aches and pains -- for example, I was having problems raising my arm, it would painfully lock up. By using the Trigger Point Therapy Workbook, I found a series of tender spots in my shoulder, and was able to do a particular type of massage that restored normal function. If someone is completely broke, go to a library, or read it in the bookstore.
