she gave me this task to fill it in , but my mind goes blank bc i don't know what is right or enough.

Yes, I've had to fill one in. Did she tell you to rate your symptoms numerically... like 1 being the worst, 10 the best?

Well, just note if there is any change in your mood (number rating) and what is on your mind at the time. Like if you become agitated by something you could make a note of that.

yes , my problem is it says here "activities" and am like a hermit bc of anxiety i stay home a lot so i don't know what to fill in :? ?
so am thinking what can i put on activities , i have an appointment today and need to hand in the form :(

Don't put anything in then - it will show a lack of activity which is accurate.

iv'e done that , so she told me she wants me to put my whole day.
like when i watch tv and what do you do after that how long do watch after that dinner etc ... :?

i need to start document every step i do , iv'e started to write things down before i forget it.


she said you can give numbers as anxiety level or the four basic emotions happiness, anger, sadness, and fear

When I had to do an activity log I wasn't doing much with myself either. It is kind of embarrassing to put really boring things down, but if they are what you are doing, I guess you have to! Here are some things that you could put down, if you do them:

Lying down
Sitting down
Sleeping
Watching TV
Zoning out
Being on the computer
Playing games
Reading
Being on the internet
Thinking
Eating

yes i do them all , thanks for the list.
she also ordered me to read for 15 min each night
and try to walk every morning outside.

I've had to do one for my bipolar disorder, I've had to keep a mood diary for a while now with what I'm doing. When I'm depressed, it's pretty hard to, but I guess watching TV or just doing nothing is well, something. If anything, it will help your doctor know how bad it's getting if you're not able to do much. It's there to help, she's not trying to shame you in any way.

Oh, and just read your last post. It's something I really need to do on a daily basis because it does help, but yes, exercise does help. It might not seem like it and you may not feel like doing it, but even a short walk on a daily basis can help you. If anything, being out in the sun should help with depression.

i will start soon with a running-therapy , where i need to run three times a week in the early morning with a group.
seems that they discovered that running and meds are a good combination for depression. (i am not on meds "yet")
