I think that might be because of the diagnostic techniques. I know in my part of the world (Eastern Europe) you have to be fairly severe to get a diagnosis. Mild cases are often regarded as "there's nothing wrong with you". Sometimes, professional would say that "it's an American thing,t o over diagnose anybody who has any signs or problems", etc.
Plus, I don't know how mental problems* are seen in the Middle East, but if it's considered shameful, not many people would be open to talk about it (or to admit they have it).
* - I don't consider this to be a mental problem, but that's how it's sometimes perceived.
