I have crush on a guy that I haven't talked to in 3-4 years and just started to talk to him about a month ago.
I'm wonder if I should tell him and get the painful relatity that he may not like me over with or wait and try to get him to do something with me for a day and then tell him.

If I do tell him how should I do it?

If he likes you, he will find a way to spend time with you.

Your idea to spend some time with him sounds like a positive route. If it seems like a correct time to let him know how you feel then you should tell him but there may come another time when you could tell him this. Starting slow, in this instance, may be a good way to begin.

*By the way, I love your avatar.

I'm still trying to get the courage to tell him, but alas I'm still to scared.
I wish there was a way to send him a note without him knowing its me.
Anyone have ideas?

Guys love it when females take the initiative.

I suggest not doing anything juvenile like an anonymous note.

Just ask him to go a movie with you and maybe dinner. If he agrees, ask him to "wear something nice" so it's obvious that it's a date and not just "hanging" out. Plus it gives you a chance to wear some makeup and/or a dress and look pretty for him.

Just get it over with. It can only bother you if you don't. I've waited up to 5 years and it was stupid of me.

Heck, I always like it if not prefer if the girl takes the initiative.
