Most people in medical school feel they need an "escape." This involves much which seems frivolous at first glance.

I don't know if I'm gifted, but I can certainly relate to having different interests that my peers. I stopped talking about science a long time ago as it don't interest peoples. For taste in entertainment and movies I was following what watched my familly, but as I discovered anime and Ghibli movies I grew further and further away from their tastes. But now I also not have the same tastes as typical anime fans; not many know about 3000 leagues in search of mother. (It could be partly because I'm from a different culture; as a french-quebeker I have seen a lot of anime while a kid (Without knowing they were from Japan; they were just cartoon for us children back then.), that will give me expectations for anime as I know about them. But as I have now zero social life outside my familly, I can't say how much my tastes are common.) I also have weird music taste, in that I mostly listen to anime and video game soundtracks and know little about the different musical groups/singers as well as the musical genres. In some way I'm even isolated in the way I view literature; most peoples think Tolkien was manichean while in true he was not. (Also I consider that the video game The shadow of Mordor is a betrayal of everything Tolkien cared for, even though most peoples seem to think this is great game and a good adaptation.)

Overall with the way I see things and my tastes I don't see how I could break my isolation.

This has nothing to do with 'gifted ness' and everything to do with social orientation or not. I don't count myself as vastly more intelligent than my peers but small talk makes me want to gouge my eyes out.

I formulated the question this way because I wanted to express my surprise at this being a problem even in academic settings. I already knew that I could only barely tolerate small talk, but I expected small talk in universities and scientific laboratories to be more... sophisticated?
