How to behave when you enter an elevator?

I go in the elevator at my GP's and I always think three things;

- What do I do if the doors refuse to allow me to exit, at least I have some books I carry around in my bag and a bottle of water.
- If this metal coffin plummets to the car park level will the force be enough to mess me up or will it just smash through the concrete and send me to the heaving inferno of the centre of the earth?
- Quick, close you stupid doors so nobody else comes in and I have to awkwardly shuffle around until we arrive at my floor!
