I used to have dark, twisted fantasies and delusional beliefs. I wonder if my dark and twisted fantasies is Schizotypal Personality Disorder.

The important thing is that you are developing the self-awareness the grow beyond them.
Do you have a goal in life to work towards? I ask because I think that having a goal and taking steps towards achieving it can help with all kinds of mental health issues.

I used to have dark, twisted fantasies and delusional beliefs, but I hid those symptoms from my family friends and doctors, because I still knew that being dark and twisted is wrong. What should I done better? Because my mental health became much worse when I hid those symptoms from them.

If you recognized those feelings were wrong and delusional, then I'd say you are well ahead of most people. You are not your feelings or your thoughts. Since you know that, you can let them pass. Just because you think something, doesn't mean you are obligated to act on it. I expect you already know that too.

I see the world in a very dark and twisted way, I am scared! I wonder if this is derealization, because I don't understand what is real anymore.

I view the world in a deluded way. There is nothing good about this world , it's full of delusional psychopathic narcissist serial killers and they are about to get me. Just look at this world, look what it's happening in Ukraine. It's ugly!

Due to my experiences in life I'd say the world is full of narcissists who are sociopathic. But I don't believe anyone is coming to kill me.

I've just experienced the majority if people not caring.

Hopefully one day you can realise no one has a motive to do you physical harm.

Could be. I was diagnosed with it before I was diagnosed with aspergers. I think they used to call it magical thinking
