Honestly it doesn't sound like he's actually trying at all. (Mind you I've only read the OP so far.) Unless he has some kind of intellectual disability as well or very severe executive dysfunction or short term memory deficit, I don't see how he has any good reason to not have made any improvement.

Whether he refuses or is incapable would make no difference to me. Unhygienic = justifiable grounds for a permanent parting of ways. That's not something I would tolerate in a partner for very long At All. Gross/unattractive/unhealthy/repulsive etc. Relationship off.

I have to agree.

I would be friends with a person with poor hygiene if they had other good qualities about them, but I'd be too repulsed by a smelly person to be romantically involved with them for very long.
