What kind of careless mistakes have you made? What was the result? What caused you to make the mistake?
Yesterday I got on the wrong train and was almost late to the land surveyors. Had to take the taxi instead of the bus and waste $25.

Attempted to duplicate my boss fake lens instead of the spare's actually prescription grade put into a new frames.
That's about 10 USD equivalent; which is at the time, worth 5 days of fulltime work salary.
There's a good chance that I've thrown out an official receipt voucher, one used for reporting taxes received from official business sections, and not the custom print kind of receipt voucher to put transactions on.
All because it 'took too much space'; turns out, replacing the whole thing costs like 500 USD equivalent.
Paying for an insurance policy after quitting work, hoping that doing so will "motivate" me to "get a bigger paying job".
Nope, that did not work and wasted like 160 USD equivalent, which is basically a quarter of my entire savings.
Just not speaking out to not take out the 3rd front tooth over a surprise dentist visit.
Taking it out was completely unnecessary. Now it's remaining posting tooth neighbors to hold the tooth bridge never felt fitted and always uncomfortable in both tooth bridge implants.
My second tooth bridge is currently broken in half and will cost me 8-10 USD equivalent per visit to put it back together. It breaks after 2 weeks of putting together.
I need a fricking tongue trainer, a diet change and possibly just eliminate all of my mouth based stimming before resigning to getting a partial denture.
Too many time and money wasted over transport mishaps all because I cannot prioritize where to go and do first tasks and next to last tasks in ways efficiently as possible. To not take like a quarter of my savings, I had to keep pissing my bosses off to avoid said mistakes.
Too many rushed based mistakes.
From losing my wallet, money, and important IDs (if it weren't for the fact that my credit card and my ATM cards are in a seperate wallet, that'll be missing too) all because of time sensitivity.
Too many penalties and fares costs for not wanting to be late. And a fare one way costs a quarter of my whole day work salary.
Might as well work for free if I exclusively travel on fare and work half day.
Just letting anyone else "own" my PC.
Lost years worth of files, the things I collected and downloaded, my own sister's thesis soft copy, and several things I really wish I still have to remember my past internet life with.

In 1986 I scraped up some money, bought a nice synthesizer, got it home, found it was damaged, took it back to the shop, got it replaced, came back, set it up on a chair, plugged it in, and accidentally knocked it onto the floor and broke it. Cracked the circuit board. Mortified, I feverishly and carefully soldered wires across the numerous breaks, and to my immense relief it worked again when I switched it back on. I've still got the synth and it still works.
Around 1973 at work, I put 300ml of concentrated sulphuric acid into a 1-litre volumetric flask and added 700ml of water. When I inverted the flask to mix the contents, the heat from the exothermic reaction cracked it and the damn stuff went everywhere. I cleaned it up as best I could but the other technicians - 3 mini-skirted women - kept getting holes in their tights from traces of acid, for weeks after the event. They gave me a lot of flak for that.
