From memory, and I could be hopelessly wrong, it was your option 1.

Maybe 2 and definately not 4.

It was 2. Lucas later added the episode number subtitles, and tried to retcon reality by denying they'd been called anything else.

(He also tried to convince everyone that the first movie had the subtitle "A New Hope" from the beginning - but I have extremely vivid memories of the first time I saw that movie, in part because it was the only time I convinced my father to actually enter a movie theater, and I can assure you that the crawl at the beginning originally read, STAR WARS (blank space) It is a dark time for the Rebellion...)

Return of the Jedi was originally titled REVENGE of the Jedi, and they changed it within weeks of its release.

No, it was changed pre-release, when Lucas suddenly decided Jedi weren't supposed to want revenge.

(I was paying attention at the time because that was just after Lucas had forced Paramount Studios to change the title of the second Star Trek film. It was going to be called The Vengeance of Khan, but Lucas threatened to sue because he was still planning on calling his third film Revenge of the Jedi - and of course the two titles are so easy to confuse, aren't they? )

All the kids in grade school just called it Empire Strikes Back and Return of the Jedi, Star Wars colloquially referred to the original movie. My Uncle recorded Star Wars off Cable TV around 1981 and sent us a copy and I do think it had the IV New Hope thing, I remember being befuddled as to why it was called part 4. In retrospect he should have just abandoned the idea of the prequels, it was a story that didn't need to be told, (like "Hannibal Rising,") especially not with THREE boring movies.

Yep, Lucas thinks he owns the right to tell other Sci Fi movie producers what they can and can't call their movies. If anyone uses the word "Empire" in their title he threatens to sue. On the commentary on the Dark City DVD, at one point they had thought of calling their movie "Dark Empire" due to there already being a 40s movie called "Dark City," Lucas' attorneys promptly served them with a lawsuit that they found pointless to spend the money and effort defending against.

And yet Lucas must have approved Star Wars XXX: A Porn Parody, and received plenty of money in return for his permission.

They we're marketed as option 2, but people just referred to them as stated in option 1.

Referring to start wars movies by episode number didn't start until episode 1 came out.
